Starting care should feel clear, simple and reassuring. We’ll guide you through each step and make sure your support is built around your individual needs.
We’ll complete an assessment
We’ll take the time to understand your needs, preferences, routines, communication requirements and desired outcomes.
We’ll create your personalised care and support plan
Your care plan will clearly set out how you want to be supported, including any health, personal care, safety or specialist requirements.
Your support begins
Once everything is agreed and the right team is in place; your care and support can begin.
We’ll continue to review your care - your support will be reviewed regularly to make sure it continues to meet your needs, choices and goals as they change over time.
